Official pay slip or payroll record of parent/guardian or the applicant (copy)

Death certificate (if applicable)

Witnessed by a religious leader (copy)

Pensions letter of retired parent/guardians (copy, if applicable)

National ID card, NHIS, or Voter’s (copy)
